For your convenience, The Cardiovascular Group participates with several insurance companies.
Please bring your insurance card, including any supplemental insurance, and present this at your time of check-in. Any co-payment required by your insurance must be made at the time of your service as you check out. Failure to pay your co-pay at the time of service will result in a $15.00 billing fee.
